Easterseals Blake Foundation (EBF) Nurse-Family Partnership (NFP) provides free, evidence-based home visits for first-time, low-income mothers, starting early in pregnancy and continuing through the child's second birthday. Nurses offer support, health education, and guidance, focusing on improving pregnancy outcomes, child development, and parenting skills.
Our Nurse Home Visitation services include multiple evidence-based programs delivered by experienced, registered nurses. While some eligibility criteria vary, your nurse will help determine which program is the best fit.
This approach ensures you get the right level of care based on your unique situation — without needing to figure it out yourself.

NFP aims to foster a secure, nurturing environment.
Registered nurses conduct home visits (or virtual visits).
First-time moms-to-be learn about nutrition, breastfeeding, and safe sleep.
Promote healthy pregnancy outcomes.
We offer flexible visits in your home or preferred location — whatever makes you most comfortable.
