Our Children’s Case Management program supports youth ages 0–17 with care coordination, psychiatric service linkage, and in-home counseling. Services include Child and Family Team facilitation, with added focus on developmental support for ages 0–5 and behavioral health planning for ages 6–17.
Care Coordination 6-17 This program provides ongoing care coordination, Child and Family Team facilitation, crisis stabilization and monitoring coordination of psychiatric services and in-home counseling and family support as needed.
Care Coordination Birth-Five
This program provides ongoing care coordination, Child and Family Team facilitation, developmental monitoring and education to help caregivers and children establish a more stable and healthy parent child relationship, coordination of psychiatric services and in-home counseling and family support as needed.
